For PESTICIDE COMPLAINTS please fill out the form below indicating “Poison Chemicals” on the “Treatment Type” dropdown of the form

AND make sure to include at least one picture of the tree crown (and any flowers or nuts if present).

In case of any issues or preferences, underneath the form is a link and a QR code which both go to the same form for sharing or any other needs.
